Patients' Rights

While you are a patient in this clinic, your Rights and Responsibilities include the following:

You have the right to considerate and respectful care.
You have the right to obtain from your health care provider complete and current information about your diagnosis, treatment, and prognosis in plain terms that you can understand. If it is not medically advisable to give this information to you, the patient, this information may be given to the designated person acting on your behalf.
You have the right to make decisions regarding medical care received, including the decision to stop treatment. If you are not able to make decisions for yourself, the legally appointed individual, next of kin, or guardian will be allowed to act on your behalf as provided by law.
You have the right to make complaints and concerns regarding treatment at the clinic. These should be made to the office manager or health care provider.
You have the right to privacy. All employees caring for you will protect your privacy and confidentiality. All treatment is confidential unless you give written permission for release of the information.
You have the right to expect the clinic to give needed health care to the best of its ability
You have the right to know if this clinic has relationships with outside parties that could affect your treatment or care.

Patients' Responsibilities

You have the responsibility to be considerate and respectful to all clinic staff.
You have the responsibility to cooperate with the staff, both office and clinical.
You have the responsibility to keep all appointments and MAY be billed for missed appointments.
You have the responsibility to follow instructions and guidelines given by the staff.
You have the responsibility to provided current information needed by the staff, including your phone number, address and insurance card.
You have the responsibility to understand what medications your are taking and why you are taken them.
You have the responsibility to make and keep follow-up appointments as instructed by the staff.
You have the responsibility to be thoughtful and considerate of everyone in the facility including other patients.
You have the responsibility to be courteous should you experience longer than expected wait times.

We have been located in our home town since 1981.

Dr. Meredith

Dr. Meredith has been in private practice since 1981. James T. Meredith, MD, PA was incorporated in 1983. He returned to Forrest City to support his hometown.

Jill Jones, APRN

Jill has been a Nurse Practitioner almost 20 years. She has worked in the area during all of those years.

Madeline Herrington, APRN

Madeline is a newly licensed APRN. She has worked as a RN for the past several years. She joins our practice on October 19, 2020.
